Post-Operative Narrow Complex Tachycardia: What is the Mechanism?
نویسندگان
چکیده
A 58 year old woman with symptomatic severe bicuspid aortic valve stenosis underwent aortic valve replacement with a mechanical prosthesis. On post-operative day four, the patient developed sudden onset of sustained narrow complex tachycardia with a heart rate of 170 180 (Figure 1). She complained of palpitations, but her blood pressure remained stable. The tachycardia was able to be briefly terminated with direct current cardioversion, revealing underlying sinus rhythm, but would recur within seconds. The tachyarrhythmia was resistant to treatment with beta blockers, calcium channel blockers, amiodarone, and digoxin and remained incessant for 48 hours. What is the rhythm, and how should the patient be managed?

A Narrow QRS Complex Tachycardia:
An 80-year-old man underwent an electrophysiologic procedure because of recurrent episodes of supraventricular tachycardia. Episodes of nonsustained narrow QRS complex tachycardia occurred spontaneously after catheter placement. Examples of the onset and termination of an episode of tachycardia are shown in Figures 1 and 2, respectively. What is the mechanism of the tachycardia? What is the lev...
متن کاملInteresting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?
Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
متن کاملTermination of a narrow QRS-complex tachycardia: what is the mechanism?
A 27-year-old man with a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length was 860 msec, and the atrial-His (AH) and Hisventricular (HV) intervals were 80 and 45 msec, respectively. A narrow QRS-complex tachycardia was induced by atrial overdrive pacing. The cycle length of the tachycardia ...
متن کاملNarrow QRS Complexes in a Patient with Baseline Surgical RBBB: What is the Mechanism?
A 26-year-old woman born with Tetralogy of Fallot initially underwent a left Blalock-Taussig shunt followed by a complete repair with a transannular patch at 2 years of age. She complained of non-sustained short episodes of palpitations. Her Holter monitor showed episodes of six beat run of non-sustained ventricular tachycardia. She was scheduled to undergo surgical pulmonary valve replacement....
متن کاملLong RP interval tachycardia. What is the mechanism?
A 41-year-old man with a history of tachycardia refractory to multiple antiarrhythmic drugs was sent to our institution. His 12-lead electrocardiogram demonstrated incessant narrow QRS complex tachycardia with negative P waves in the inferior leads and long RP interval. Occasionally, the tachycardia terminated after a P wave and then was restarted after a sinusal beat. An EP study was performed...
